The Mobility Brief

This month, The Mobility Brief examines key points in the continuum of patient care, from system-level implementation to specific clinical interventions. Work from Manuel Montero-Odasso considers the organizational hurdles in putting global fall prevention guidelines into practice, drawing lessons from Sweden. E. Ali Bateman outlines a protocol to review strategies for physician-to-physician handover between acute care and rehabilitation. The issue also includes a systematic review from Janelle Unger on the effectiveness of functional electrical stimulation for walking outcomes in people with incomplete spinal cord injury, and a case report from Eldon Loh describing a modified nerve block injection for a female with persistent pain after knee replacement.

does training with electrical stimulation help people with an incomplete spinal cord injury walk faster or for longer?
It is unclear if a therapy combining functional electrical stimulation with locomotor training improves walking for individuals with a motor incomplete spinal cord injury. Researchers addressed this question by reviewing and analyzing thirteen studies that assessed the therapy's effects on walking speed and endurance.

What are the known ways to improve patient handovers between acute and rehabilitation care?
Poor communication between physicians when patients are transferred from acute care to rehabilitation can lead to medical errors and adverse health outcomes. Researchers performed a comprehensive search of multiple databases to identify and analyze existing studies that describe strategies to improve these handovers.

When a nerve block for knee replacement pain fails, could an injection in a different place offer relief?
Persistent pain following total knee arthroplasty is a challenging condition to manage. This case report describes an 84-year-old female patient who received diagnostic nerve blocks with a classical injection technique and, after that failed, with a modified protocol using different injection sites.

Why do accepted health guidelines fail in practice?
Putting global guidelines for fall prevention into daily practice is often challenged by organizational-level barriers. To address this, a study in Sweden brought together researchers, practitioners, and individuals with lived experience to co-create a knowledge support tool across various care settings.
